Requirement(s) of Order:     Transfer in writing, ownership of the water system to Lumpkin County Water and Sewerage Authority providing all necessary documents including, adequate legal description, bill of sale/warrenty deed; Lumpkin County shall abide by following conditions: submit as-built plans prepared by professional engineer, as required, 2)submit approvable Application For A Permit To Operate A Public Water System, 3)provide proper documentation from Phase I, Inquiry and Discovery checklist, within 30 days executed order; provide Phase II, Technical Review and Phase III, Permitting and Contract Services checklists within 120 days; upon approval, complete system construction of modifications, submitting proper certification; obtain permit to operate a public water system within 12 months, or cease operation

Cause of Order: Deadline extension to complete sewer system improvements  
Requirement(s) of Order:     Complete all phases of master sewer system improvements construction, including collecting/transmitting entire wastewater flow to Macon Water Authority system by September 30, 2012; terminate all wastewater flow to existing treatment pond; construction shall be completed by EPD approved plans; submit written response to EPD stating all wastewater flow has been eliminated from current facility, and include detailed description of all sewer connections to other municipalities; complete closure of present facility including abandonment procedure submitted to EPD for review, and conversion studies used to alter pond's existing use; develop/implement measures to prevent violations of Permit effluent parameters at current facility; stipulated penalties

Location: Mcintosh County
Order Number: AMENDMENT TO EPD-WQ-5333
Date of Issue: January 17,2012
Cause of Order: Schedule revision for compliance with City of Darrien's Sewer Use Ordinance regarding industrial waste  
Requirement(s) of Order:     Within 120 days Mr. Chuang will obtain determination from the City that industrial waste being discharged to POTW is in compliance with Sewer Use Ordinace and is not in violation of any pretreatment standard or requirement; Mr. Chuang has within 150 days to submit application for a Pretreatment Permit if City has not provided a compliance letter; provide Notice of Intent to obtain coverage under the General NPDES Permit for Storm Water Discharges Associated with Industrial Activity; effective date of all conditions in this order is based on original execution date of August 25, 2011; all other conditions of original order have not changed
