Cause of Order: Violations of Air Operating Permit/including failure to: report, continuously monitor, maintain a total resource effectiveness index value, conduct performance test in timely manner  
Requirement(s) of Order:     Submit proposed Supplemental Environmental Project, including detailed cost estimate of installation/construction schedule; implement SEP, submitting any required permit applications; provide descriptive quarterly progress reports; submit detailed installation/implementation completion report including estimate of achieved emissions reduction; if proposed SEP is not acceptable or DSM fails to install approved SEP by Dec. 31, 2011, DSM shall pay additional $1, 216,000.00; pay all underpaid fees; install proper air pollution control device, submitting operation permit & conduct performance test; payment for excess VOC emissions starting January 1, 2010; submit approvable quality modeling plan & conduct AQ modeling of NSR pollutants; submit approvable operating plans, as required
