Under authority of the Hazardous Waste Management Act :

Order Number: EPD-HW-1796
Cause of Order: Violations of Rules for Hazardous Waste Management/failure to properly label (4) oil drums; failure to clean-up/manage released used oil  
Requirement(s) of Order:     Submit analytical results of background sample; upon approval, properly remove/dispose of all contaminated soil; make hazardous waste determinations; provide copies of manifests or disposal tickets of excavated soils, and also submit documentation that proper contamination removal/disposal has been completed

Cause of Order: Violations of Permit to Operate a Public Water System/continuous failure to monitor; failure to prepare & distribute Consumer Confidence Reports  
Requirement(s) of Order:     Make necessary repairs to treatment plant ensuring detectable residue of free chlorine 0.2 or more parts per millions; maintain contract with proper certified water operator; maintain complete/correct system records, including proper monthly groundwater reports, signed by operator; collect/have analyzed treated water samples for disinfection byproducts, as required; complete requirements for public notification; distribute accurate CCRs for 2009-2010; provide proof of valid lab contract and that all fees have been paid; stipulated penalties or requirements
